ACCIDENT ANALYSIS COMMITTEE

1.             AIR SAFETY TRENDS

1.2           Trend Analysis System

1.2.1         Aviation Safety Reporting Systems (Airlines, etc)

To monitor proposals by airlines and other corporate bodies to establish programmes designed to solicit written pilot reports on a spectrum of observed operational problems and safety deficiencies, e.g. the AA Safety Action Partnership Programme (ASAP).

The purpose of the next two agenda items is to highlight specific airplane and helicopter accidents or serious incidents, monitor the investigation, review the final report and identify trends or issues that need IFALPA involvement either through direct action from the AA Committee or through referrals to other Committees.

2.             REVIEW OF ACCIDENTS

2.1           Miscellaneous Recent Accidents

3.             REVIEW OF INCIDENTS

3.1           Miscellaneous Recent Serious Incidents

3.2           Airproxes

4.             FLIGHT RECORDERS

To review technical standards for Flight recorders and to monitor the work of the ISASI/EUROCAE Working Groups on FDR/CVR specifications.
In conjunction with LEG and IND Committees.

4.1           Misuse of Airborne Recorded Data

To identify and list all events where airborne recorded data are used for purpose other than investigation under ICAO Annex 13 and accident prevention.

4.2           Cockpit Image Recorders.

To monitor the development of Cockpit Imaging Equipment for the Flight Deck. IFALPA opposes their installation until adequate safeguards and protection of the images to be used only in accident/serious incident investigation are guaranteed.

5.             ICAO ANNEX 13 AND RELATED GUIDANCE MATERIAL

5.1           ICAO Annex 13

5.2           ICAO Manual of Aircraft Accident and Incident Investigation (Doc 9756, former doc 6920)

5.3          ICAO Accident Prevention Manual (Doc 9422)

5.4           The work being covered by other Panels, Working Groups and Study Groups

5.5           Training guidelines for accident investigators

6.             IFALPA ANNEX 13 AND OTHER RELATED ISSUES

6.1           IFALPA ANNEX 13

Continue to review for possible amendments that the Committee deems to be necessary.

6.2           Criminal Liability of Pilots Involved in Accidents (CLOP)

To identify cases where pilots have been judged criminally liable following an accident.

6.3           HUPER Policy

To provide guidelines for Accident Investigators when examining the human factors aspects of accidents.  How to ensure that Investigators take into account the policies and principles as presented by the HUPER Committee.

6.4           Implementation of Policy (Flight Data Analysis, Non-Punitive Culture, etc.)

To identify to the Principal Officers any specific AA-related policies which require special implementation action.

6.5           IFALPA Accident Response Plan

To review/draft the IFALPA Accident Response Plan.

7.             IFALPA ACCREDITED ACCIDENT INVESTIGATOR SCHEME.

To continue to review the organisation and operation of the IFALPA Accredited Accident Investigator Scheme.  Accredited investigators should view AA Committee meetings as continued training and should endeavour to attend as often as possible.  New guidelines on re-accreditation were proposed in 2004.

7.1           Recent Accreditation Approvals

To update the Committee on accreditations approved since the last meeting.

8.             COMMUNICATIONS

8.1           Hot Topics

To maintain a pool of “hot topics” principally to be used for future InterPilot articles on subjects where a possible safety trend has been identified by the Committee.

8.2            Hit List

The hit list was introduced during 1996 with an initial group of five safety issues regarded as having the highest priority.  This list was reviewed in June 2004. Current items are:

A.         CFIT & Approach and Landing Accidents.

B.         Maintenance-related accidents or incidents.

C.         Safety Information Exchange.

D.         Human Factors: Training and Design.

E.         Runway Incursions.

F.         Flight Data Analysis.

8.3           Tracking of Referrals

To update the Committee on referrals made to/received from other Committees.

9.             SAFETY TRENDS DATABASE

This database is meant to be a reference for safety items and/or trends the committee has discovered. The purpose is to list accidents/incidents discussed by the Committee and keep them for further reference.
